=Key message:
Diabetic retinopathy begins earlier than we think.
Even Stage 0 diabetic eyes—before clinically visible DR—already show:
• Reduced vessel density (VD), skeleton density (SD), and fractal dimension (FD)
• Increased vessel diameter index (VDI) and acircularity index (AI)
=What makes this study different:
• Prospective design across multiple DR stages (Stage 0–3)
• Longitudinal follow-up over 2 years
• Combined retinal and choroidal vascular analysis
• One of the few studies demonstrating prospective choroidal changes, not just cross-sectional snapshots
=Disease progression was characterized by:
• Decline in SFCT and CVI
• Worsening superficial vascular density
• Progressive vascular dilation and irregularity
🎯 Take-home:
Quantitative OCTA metrics—especially deep VDI and CVI—may serve as powerful biomarkers for:
• Early detection
• Disease staging
• Monitoring progression
We may be closer to detecting DR before it becomes clinically visible.
